Assessment 1
Heart disease is the #1 killer of men and women in the US, but many cases are preventable. There are important steps you can take to lower your risk.
This interactive tool estimates your risk of heart attack and stroke. Anyone can use it. If you have heart disease, be sure to talk to your doctor about your risk.
1.Get to the assessment page by clicking on the link below.

2. Begin the Heart Disease Risk Calculator by entering your information in the box on the left side of the page.
3.You will be asked to enter your height in centimeters and you weight in kilograms:
If you do not know these metric number equivalents, but know them in feet and inches and pounds – you can visit the links below to calculate your height in centimeters and weight in kilograms.

4. After you have finished entering your information, you click on Calculate Risk button. Then you will be provided a 10-year risk percentage.
5. Take a screenshot (as in the example below) of your full results (save as a .png, .jpeg, or .pdf).
6. Submit your saved screenshot (.jpeg, .png, or .pdf) for 10 points.
Assessment 2
1. Go to: http://www.e-tarocchi.com/esteem/es.phpLinks to an external site.
2. Start the self esteem test by clicking “Begin Assessment”
3. Enter your name and select either “True” or “False” for each statement. The system has repeated forms of questions to determine if you actually submitted an honest response.
4. After you select the final response. Click “Show Me My Results”
5. If you have just left all the answers marked “True” in an effort to skip actually attempting the assessment, the system will score you 42 percent and you will not receive credit. The assessment has repeated questions and can detect a lack of tru effort. So please take the time to complete it honestly.
6. You must submit a .pdf or .jpg of your results**. For full credit, it must include screenshot of:
the header with your name
the graph, and
your score.
**See the example above in #5. Your submission should not have the “red highlighted paragraph”.
7. Project is worth 10 points.
