Choose one (1) adult female patient presenting for a women’s health issue during your practicum experience to analyze for the Case Analysis Exercise Assignment.
Follow the following format:
Introduction: Discuss (past tense) the reason for selecting the patient encounter. Briefly discuss Practicum Objectives that encounter assisted in meeting for this practicum experience. 1 – 2 Paragraph(s).
SOAP: Replicate SOAP from case encounter. If the replicated SOAP is not included, the assignment will be returned to the student for completion and resubmission with 10% grade deduction.
Differential Diagnoses: Develop a comprehensive list of differential diagnoses and rank according to the highest index of suspicion. There should be a minimum of 4 differential diagnoses.
For each differential diagnosis provide the following:
1. Epidemiology: Discuss the incidence, prevalence, and at-risk populations for each differential diagnosis.
2. Etiology and Pathophysiology: Provide a 1 paragraph synopsis of the etiology and pathophysiology for each differential diagnosis.
3. Clinical Manifestation: Discuss the common pertinent positive and negative history and physical exam findings that would be associated with the differential diagnosis.
4. Diagnostics: Present the initial diagnostic test that are appropriate to inform the differential and the rationale for each diagnostic test. Provide evidence that supports the indication and appropriateness for each diagnostic test. Provide the risks and benefits of each diagnostic test. Discuss the cost effectiveness of the chosen diagnostic tests compared with other potential diagnostic tests.
Final Assessment(s): Create and include a table that lists each of the differential diagnoses along with pertinent positives and negatives. Present the final diagnosis discussing the diagnostic reasoning and process which contributed to the formulation of the final assessment. If appropriate, provide all current diagnosis and relevant past diagnosis in order of priority.
Management: Provide a narrative of the plan of care for the final assessment. The plan of care should be supported by current evidence (include citations and level of evidence) for each component of the plan. Provide the following:
1. Include if additional diagnostic tests are indicated.
2. Non-pharmacological and pharmacological therapies.
3. Referrals, counseling and education, health care maintenance.
4. Follow up.
5. Short-term and long-term outcomes associated with the plan of care.
Health Promotion and Counseling: Discuss and provide an evidence-based rationale of appropriate risk assessments/preventative screenings for the patient considering their age, race, gender, family history, and/or health status. Discuss important topics for health promotion and counseling.
Patient/Family Education: Discuss patient/family education pertinent to the care of the patient, consider health literacy of the patient and rationale for appropriate education.
Christian Ethics Applied to Patient Care: Discuss the FNP role in managing the patient presenting with the acute episodic condition within context of the Christian worldview. Consider ethical principles that underpinned patient care and/or used to resolve an ethical dilemma encountered in the care of this patient.
Level of Evaluation and Management: Provide the level of evaluation and management that this patient encounter generated. Discuss how the level of management was justified according to the CMS Coding and Billing Guide.
Format and Presentation:
